They aim to support grantees and the various target audiences to strengthen their integrated system of care, engage patients in their own healthcare and teach them how to stay health and manage their health conditions, and provide consideration for culture, language and promote overall positive health outcomes through the BH2I.

Use of Local Evaluation Findings in Integrated Care

Data and program evaluation findings are critical for leaders, yet sometimes in Native communities we use intuition and word-of-mouth reactions to programs instead of data to inform decision making. In other words, some integrated care (IC) programs don’t systematically collect any data. Others collect local evaluation data, but they initiate the local data collection process too late to gather sufficient data.

Assessment for Substance Abuse Treatment Readiness

When making the decision if an individual is a candidate for substance use disorder services it may be advantageous to understand an individual’s treatment readiness. One instrument that is commonly used to assess treatment readiness is the Stages of Change, Readiness, and Treatment Eagerness Scale (SOCRATES).
