A Roadmap for Integrated Care
The BH2I-2 Standards of Care Manual outlines an evidence-based, culturally grounded framework for integrating behavioral health care with primary care. Designed to help I/T/U clinics advance along the integrated care continuum, the manual provides practical guidance, tools, and considerations across ten domains of integrated care.
The framework is built on empirical research and lessons learned from BH2I Cohort 1 grantees, with subject matter expertise contributed by representatives from the Indian Health Board of Minneapolis, Muscogee (Creek) Nation, and Yellowhawk Tribal Health Center. It adapts and Indigenizes the Comprehensive Healthcare Integration (CHI) Framework, with added domains for telehealth and integrated service.
