Montefiore Framework and the Continuum of Care Model

The Montefiore Framework is a guide that helps primary care practices increase their ability to serve patients with mental health issues within their primary care settings. This webinar will provide an overview of the framework and participants will learn how to apply the domains in their practice setting.

1.Examine the 8 domains of integrated care including the key components and subcomponents
2. Identify evidence based practices to reach higher levels of integrated care.
3. Utilize the Monefiore framework to determine your clinics current level of integrated care practice
